Over the past few year travel medicine has become increasing specialised, we are finding that patients are travelling to ever more exotic locations. There may be a number of complicating factors and failure to cover all aspects could nowadays be a potentially serious issue. For this reason, in accordance with current guidelines we request that all patients complete our Travel Vaccination Questionnaire. By clicking on the link below, you can complete and submit it online, or you can print the form off and return it to Reception.
There is further information about countries and vaccination required on the links below.
Travel Vaccination Questionnaires should be completed preferably 2 months before your departure date. Vaccines need to be given in plenty of time in order to take effect in the body and offer protection. Also some vaccines require a course of injections, which are separated by a number of weeks and not frequently used vaccines will need to be ordered in. Vaccines can be given closer to your travel, this is better than not to have them at all, but they will not offer the full protection.
You should allow at least 1 week between handing in your completed form and your appointment with the Practice Nurse, to enable the Nurse enough time to look through your questionnaire and find out what vaccinations you need. Depending on your requirements, the Nurse may contact you prior to your appointment to discuss this.
